Medical Records Requests

To receive copies of your medical records, you must prepare a request in writing. Please include your full name, date of birth, current phone number, and the date of service. Please tell us where you would like us to send the copies and be sure to include your signature.

Birth certificates are obtained through the county Office of Vital Records. If you or your baby were born at Summit, Merritt, Providence or Peralta, call (510)272-6363 for further instructions.
