Please be sure to describe the details of the care, services or interactions you had as a patient or visitor. If you have the full name of the individual you’d like to thank, please feel free to include it. All information you provide will be kept confidential.
Based on the information you provide, we will route your message to the appropriate department or person within five business days of receiving it. A copy of your note will be sent to the appropriate staff, department manager, and to other hospital leadership. Again, the information you share will be kept confidential in compliance with our medical center Privacy Policy.
